In a close shave for passengers, two coaches of 15269 Muzaffarpur-Sabarmati Jan Sadharan Express derailed at Kanpur-Tundla section in Prayagraj Division, confirmed the Indian Railways. The public transporter said that no casualties were reported in the incident. According to railway officials, the 6th and 7th coaches from the engine derailed on the loop line of the station.

Earlier on July 19, 2024, a passenger train got derailed in Uttar Pradesh killing at least two passengers and injuring 20 others. The train was on its way to Assam's Dibrugarh from Chandigarh when it derailed near the town of Gonda in Uttar Pradesh. At least six coaches were derailed and two had overturned.

In June last year, a tragic collision occurred in West Bengal when a cargo train slammed into a stationary passenger train, leaving nine people dead and injuring dozens more. Investigators later confirmed that the cargo train's driver—who also lost his life in the crash—had ignored a stop signal, leading to the deadly accident.

In November 2024, the Ministry of Railways had said that as a consequence of various safety measures taken over the years, there has been a steep decline in the number of accidents. "Consequential Train Accidents have reduced from 135 in 2014-15 to 40 in 2023-24. The causes of these accidents broadly include track defects, Loco/Coach defects, equipment failures, human errors etc. It may be noted that the consequential train accidents during the period 2004-14 was 1711 (average 171 per annum), which has declined to 678 during the period 2014-24 (average 68 per annum)," the public transporter had said.

It further added, "Another important index showing improved safety in train operations is Accidents per Million Train Kilometer (APMTKM) which has reduced from 0.11 in 2014-15 to 0.03 in 2023-24, indicating an improvement of approx.73% during the said period."
